Becoming a patient at Santé Ouest

Here are the steps to follow if you want to become a patient at Santé Ouest.

1. Register to Health Connect Registry (HCR) 

Health Connect Registry is the provincial system established by the BC Ministry of Health to help people who do not have a family doctor or nurse practitioner find a care provider near their home. 

Register for the Health Connect Registry by filling out the online form. To the question “Do you need a translator?”, select “Yes” and then indicate “French” as the desired language.

Processing times vary depending on the situation. We are unable to provide an estimate of the waiting time.

Important: Registration with the HCR does not guarantee a connection to Santé Ouest. The system directs you to a healthcare provider (family doctor or nurse practitioner) according to availability and your criteria, including language and place of residence. You could therefore be directed to another health center, another clinic, or an independent French-speaking provider. 

2. Contact Santé Ouest

If you wish to become a patient at Santé Ouest, we recommend that you contact us by email while registering for the HCR and let us know about your interest. 

We will send you a registration link as well as two forms to fill out: 

  • A consent form for the center’s services 
  • A medical information form

These two forms are required to complete your registration. Without them, we will not be able to create your patient file. 

If you register your whole family 

The registration link allows you to create a profile for each adult. A separate email address is required for each profile. 

Children without email addresses can’t create their own profiles. Our administrative team will create their file and link it to the profile of one of the parents. 

However, each member of the family, adult or child, must complete their own consent form and medical information form. 

Due to high demand, your name will be added to our waiting list upon receipt of your completed forms. We will contact you as soon as a spot becomes available. Thank you for your patience.

3. First appointment with your healthcare provider

Once your forms are received and processed, our team will contact you to schedule a first appointment with a family doctor or nurse practitioner.

At the end of this first consultation, and with your agreement, you will officially be attached to your healthcare provider.

4. Welcome to Santé Ouest

You have access to all the services offered by our multidisciplinary team. Depending on your needs, you can schedule an appointment with the professionals concerned by contacting our office or directly through your patient plateform.

Frequently asked questions

No. We do not accept walk-ins. Once connected via the Health Connect Registry, our team will contact you to set up an initial face-to-face appointment. Subsequently, patients already connected to a healthcare provider will be able to book an appointment online via this site.

Yes, our medical team takes on new patients. To do so, you need to register on the Health Connect Registry system. It is important to specify the French language in the preferences. For more information about registering with the Registry or updating your file, please visit the dedicated page on the RésoSanté C.-B. website ("Health Connect Registry" tab).

We are open Monday to Friday from 09:00 am to 5:00 pm, by appointment only.

Whether virtual or in-person, most services offered at Santé Ouest are covered by the MSP (Medical Services Plan).

Please note, however, that some specific medical or nursing services may not be covered. Don't hesitate to ask us for more information.

Yes! We offer most routine vaccines, as well as flu and COVID-19 vaccines, free of charge or on prescription depending on eligibility criteria (age, risk factors, etc.).

For paid or travel vaccines, you can get them in the pharmacy, and our nurses can administer them with pleasure.

SkyTrain construction work is causing major disruption to traffic and parking around West Broadway, whatever your mode of transport.

If you're coming by car, choose the streets north of West Broadway (8th, 7th, etc.), where you'll find free parking (2 hrs).

To get to Santé Ouest, walk along West Broadway from Maple Street on the north side.

Santé Ouest is located at 2025 W Broadway, at the corner of Maple Street. Bus 99 takes you to the Broadway @ Arbutus stop, a 3-minute walk away.

A community health center (CHC) is a non-profit organization that provides basic health care and health promotion programs adapted to the needs of local communities.

The operation of a community health center is based on several key principles, aimed at providing accessible, patient-centered health care tailored to each individual's specific needs. A team of providers will work with the patient to determine his or her goals of care, and ensure that he or she has access to the services and support he or she needs to achieve his or her health and well-being goals.

No. The Santé Ouest francophone community health center offers bilingual services in French and English. Although the center is open to all, certain groups will be prioritized, notably those without a family doctor or those belonging to vulnerable groups. Health care will be offered in person and virtually, helping to reduce inequalities in access to health care.

Call 911 in case of a life-threatening emergency: serious accident, fire, heart attack, breathing difficulties, immediate danger, etc. Emergency services (ambulance, fire, police) will respond quickly.

811

Dial 811 to reach HealthLink BC. You can speak with a: 

  • Nurse, available 24/7, for general health advice,
  • Pharmacist, evenings and overnight (5 pm - 9 am), for questions about your medications,
  • Dietitian, Monday to Friday (9 am - 5 pm), for nutrition advice. 

Interpretation services are available, including in French.

In summary:

  • 911 = immediate emergency, life-threatening situation, or urgent need for help.
  • 811 = non-urgent health advice, information, guidance, medications, and nutrition.
