Family doctor and nurse practitioner
The family doctor or nurse practitioner is an important contact person in your health journey at Santé Ouest. He or she ensures the overall monitoring of your health, assesses your concerns, establishes diagnoses, prescribes treatments and coordinates your care when necessary.
- A one-time or persistent health problem.
- A regular medical follow-up.
- The diagnosis and treatment of acute or chronic diseases.
- The renewal of a prescription.
- A mental health consultation.
- Preventive care and screening.
- A reference request to a specialist.
- Medical certificates or health forms.
- Minor care (wounds, warts, swabs, etc.).
The first appointment with a doctor or nurse practitioner must be made by phone with our reception team in order to schedule a consultation time tailored to your needs.
Subsequent appointments can be made via the patient platform or by phone.
Aythorized Nurse
At Santé Ouest, licensed nurses are an integral part of your healthcare team. Depending on your situation and health needs, it is possible that your appointment will be scheduled with an authorized nurse rather than with your family doctor or nurse practitioner. Their role is to assess your needs, ensure certain clinical follow-ups, offer preventive care and support you in managing your health, in collaboration with the entire team.
- A health assessment or clinical follow-up.
- The monitoring of chronic diseases (diabetes, hypertension, asthma, COPD, etc.)
- A vaccination or an injection.
- Wound care.
- STBBI screening or a Pap test.
- Sexual health issues.
- Prenatal or postpartum follow-up.
- Mental health or substance use support.
- Advice on lifestyle habits, physical activity or quitting smoking.
- A referral to appropriate resources or services.
Appointments with an authorized nurse can be made via the patient platform or by phone. For a new health problem, a concern requiring an assessment, or a situation where you don’t know which professional to consult, It is preferable to communicate directly with our team by phone so that we can direct you to the person best suited to your needs.
Social worker
Social workers offer support when personal, family, social or emotional difficulties impact your health and well-being. They support you in your efforts and help you access the resources adapted to your situation.
- Stress, anxiety or a feeling of exhaustion.
- A mourning or an important transition period.
- Adaptation to a chronic disease or new diagnosis.
- Family or relationship difficulties.
- A sense of social isolation.
- Concerns related to housing, finances or access to services.
- Administrative procedures or applications for benefits.
- A situation of crisis or vulnerability.
- A need for referral to community resources.
You can contact us directly, whether or not you have already met with a professional from Santé Ouest. Send us an email briefly describing your needs and specifying your contact details. A social worker will then contact you.
Clinical Counselor (RCC)
The clinical counselor offers short-term individual therapy services for people experiencing emotional, relational, or psychological difficulties that affect their daily lives. His/her support is centered on your needs, your objectives and your strengths.
- Anxiety, stress or depression.
- A trauma or a difficult event.
- A chronic pain.
- A mourning or a significant loss.
- Relational or family difficulties.
- A period of change or uncertainty.
- A decline in self-esteem or sense of security.
- A crisis situation requiring psychological support.
Access to this service is based on the referral of a doctor, nurse or social worker from Santé Ouest. After evaluating the request, our team will contact you to schedule a first appointment.

If you do not have access to your patient portal or are having trouble logging in, contact us by phone or email. Our team can help you activate or recover your access.

Frequently asked questions
No. We do not accept walk-ins. Once connected via the Health Connect Registry, our team will contact you to set up an initial face-to-face appointment. Subsequently, patients already connected to a healthcare provider will be able to book an appointment online via this site.
Yes, our medical team takes on new patients. To do so, you need to register on the Health Connect Registry system. It is important to specify the French language in the preferences. For more information about registering with the Registry or updating your file, please visit the dedicated page on the RésoSanté C.-B. website ("Health Connect Registry" tab).
We are open Monday to Friday from 09:00 am to 5:00 pm, by appointment only.
Whether virtual or in-person, most services offered at Santé Ouest are covered by the MSP (Medical Services Plan).
Please note, however, that some specific medical or nursing services may not be covered. Don't hesitate to ask us for more information.
Yes! We offer most routine vaccines, as well as flu and COVID-19 vaccines, free of charge or on prescription depending on eligibility criteria (age, risk factors, etc.).
For paid or travel vaccines, you can get them in the pharmacy, and our nurses can administer them with pleasure.

SkyTrain construction work is causing major disruption to traffic and parking around West Broadway, whatever your mode of transport.
If you're coming by car, choose the streets north of West Broadway (8th, 7th, etc.), where you'll find free parking (2 hrs).

A community health center (CHC) is a non-profit organization that provides basic health care and health promotion programs adapted to the needs of local communities.
The operation of a community health center is based on several key principles, aimed at providing accessible, patient-centered health care tailored to each individual's specific needs. A team of providers will work with the patient to determine his or her goals of care, and ensure that he or she has access to the services and support he or she needs to achieve his or her health and well-being goals.
No. The Santé Ouest francophone community health center offers bilingual services in French and English. Although the center is open to all, certain groups will be prioritized, notably those without a family doctor or those belonging to vulnerable groups. Health care will be offered in person and virtually, helping to reduce inequalities in access to health care.

Call 911 in case of a life-threatening emergency: serious accident, fire, heart attack, breathing difficulties, immediate danger, etc. Emergency services (ambulance, fire, police) will respond quickly.

Dial 811 to reach HealthLink BC. You can speak with a:
- Nurse, available 24/7, for general health advice,
- Pharmacist, evenings and overnight (5 pm - 9 am), for questions about your medications,
- Dietitian, Monday to Friday (9 am - 5 pm), for nutrition advice.
Interpretation services are available, including in French.
In summary:
